Anemia in children: Iron Deficiency vs Thalassemia Trait (common in our region)—symptoms, tests, and foods that help

Anemia is one of the most common health concerns seen in children across Dubai and the UAE. While it may seem mild at first, untreated anemia can affect a child’s growth, learning ability, immunity, and overall development.

In our region, the two most frequent causes of anemia in children are iron deficiency anemia and thalassemia trait. Although they may look similar on initial blood tests, they are very different conditions and require different approaches to management.

Understanding the difference helps parents take the right steps to protect their child’s health.

What is anemia in children?

Anemia occurs when a child does not have enough healthy red blood cells or hemoglobin to carry oxygen throughout the body.

Children with anemia may feel tired, weak, or short of breath because their tissues are not getting enough oxygen to function properly.

In Dubai’s multicultural population, pediatricians frequently see anemia caused by:

  • Nutritional deficiencies (especially iron)

  • Genetic blood conditions such as thalassemia trait

Iron deficiency anemia vs thalassemia trait

Although both conditions can cause low hemoglobin levels, their causes and treatments are very different.

Iron deficiency anemia

This is the most common cause of anemia in children worldwide.

It occurs when a child does not get enough iron from their diet or loses iron faster than it can be replaced.

Common causes include:

  • Poor dietary intake

  • Excessive milk consumption in young children

  • Rapid growth phases

  • Poor iron absorption

Thalassemia trait

Thalassemia trait is a genetic condition, not caused by iron deficiency.

It is particularly common in the Middle East, South Asia, Mediterranean, and African populations.

Children with thalassemia trait:

  • Usually have mild anemia

  • Are otherwise healthy

  • Do not benefit from iron supplements unless iron deficiency is also present

This is why correct diagnosis is essential.

Common symptoms of anemia in children

Many children show subtle symptoms that can easily be overlooked.

Signs to watch for include:

  • Pale skin or lips

  • Constant tiredness or low energy

  • Poor concentration or learning difficulties

  • Shortness of breath during activity

  • Frequent infections

  • Poor appetite

  • Delayed growth in severe cases

If these symptoms persist, a medical evaluation is important.

How doctors diagnose anemia

A simple blood test can help identify the cause of anemia.

Common tests include:

  • Complete Blood Count (CBC) – checks hemoglobin levels and red blood cell size

  • Serum ferritin – measures iron stores in the body

  • Iron studies – assess iron absorption and availability

  • Hemoglobin electrophoresis – detects thalassemia trait and other hemoglobin disorders

In Dubai, pediatricians often request hemoglobin electrophoresis early due to the high prevalence of thalassemia trait in the region.

Foods that help prevent and manage iron deficiency

Diet plays a key role in preventing iron deficiency anemia.

Iron-rich foods for children include:

  • Red meat, chicken, and fish

  • Egg yolk

  • Lentils, chickpeas, and beans

  • Spinach and dark leafy greens

  • Fortified cereals and grains

  • Dates and raisins

Tip: Vitamin C improves iron absorption. Pair iron-rich foods with:

  • Oranges

  • Strawberries

  • Tomatoes

  • Bell peppers

Avoid giving tea or excessive milk with meals, as they reduce iron absorption.

Important note about iron supplements

Iron supplements should only be given after medical confirmation.

Giving iron to a child with thalassemia trait who does not need it can lead to iron overload, which may harm the liver and other organs.

Always consult a doctor before starting supplements.

When anemia needs medical attention

You should consult a doctor if your child has:

  • Persistent fatigue or weakness

  • Poor growth or developmental concerns

  • Recurrent infections

  • Abnormal blood test results

  • A family history of thalassemia or anemia

Early diagnosis ensures proper treatment and avoids unnecessary medication.
